Email template to transfer a prescription to another pharmacy
You are moving, changing insurance, or simply tired of a pharmacy that is always out of stock, and you want your prescriptions filled somewhere else. In most cases the new pharmacy does the work: it calls the old one and pulls the record over. What slows it down is missing information, so sending the request in writing with everything spelled out saves two or three phone calls. Controlled substances, such as many pain and ADHD medicines, usually cannot be transferred at all and need a fresh prescription from your doctor, so treat those separately.
Before you send
- The name, address, and phone number of the pharmacy you are leaving, and the one you are moving to. The new pharmacy needs both to make the call.
- Each medicine by name and strength, for example "metformin 500 mg," plus the prescription number from the label if you still have the bottle.
- Your date of birth and the phone number on file, which is how the pharmacy finds your record.
- Your insurance card or discount card details, if the new pharmacy does not already have them.
- A note of which medicines you are low on, so they know what to fill first.

Everything in brackets is yours to fill in. Keep the rest as it is or change the tone to match how you write.
After you send
- Most transfers are done within a business day. If nothing has happened in two days, call the new pharmacy and ask whether the old one answered.
- Controlled substances are the usual sticking point. If the pharmacy says one cannot be transferred, ask them to tell you which, so you can request a new prescription from your prescriber right away.
- Do not let the last few pills run out while you wait. If the new pharmacy is stalling, ask the old one for one more fill to cover the gap.
- Once the first fill goes through at the new pharmacy, check the label against the old bottle. Strength and directions should match exactly.
