Email template to report a prescription error to a pharmacy

You picked up a prescription and something is wrong: the wrong medicine, the wrong strength, the wrong directions on the label, or a quantity that does not match what your doctor ordered. Deal with the safety part first by calling the pharmacy and your prescriber, and do not take another dose until someone confirms what you have. Then put it in writing. A written report gets the mistake into the pharmacy's own records, which is what triggers an internal review, and gives you a document if the error caused you harm or cost.

Before you send

  • The bottle or package, photographed, with the label readable. Do not throw it away, even after they replace it.
  • What your prescriber actually ordered, and how you know, for example the visit summary or a call to their office.
  • Dates and names: when you picked it up, who handed it to you if you know, and how many doses you took before noticing.
  • Any symptoms you had, and whether you contacted your doctor or a nurse line about them.
  • What you want: the correct medicine at no charge, a refund of what you paid, and a written explanation.

I picked up prescription [prescription number] for [your full name] on [date]. What I received was [what was on the label and in the bottle], but my prescriber ordered [what was actually prescribed].

I took [number of doses] before I noticed, on [dates]. [I have had no symptoms, or: I experienced the following and contacted my doctor on date: symptoms.]

I still have the bottle and label and have not discarded them.

Could you please:

- Confirm what your records show was prescribed and what was dispensed
- Provide the correct medicine at no charge
- Refund the [amount] I paid for the incorrect fill
- Tell me in writing what caused the error and what was done about it

Everything in brackets is yours to fill in. Keep the rest as it is or change the tone to match how you write.

After you send

  • A responsible pharmacy replies quickly, replaces the medicine, and refunds you. Errors are reported internally and often to the state board.
  • If the manager is slow or dismissive, take it to the chain's patient safety or corporate pharmacy line, and then to your state board of pharmacy, which licenses pharmacists and accepts complaints from the public.
  • Keep the bottle, the label, and every reply. If the error caused a medical problem, those are the evidence.
  • If you were harmed, talk to a lawyer before signing anything the pharmacy sends you, including any offer that asks you to release claims.
